What do I need to bring to my interview with Unified Care?

In order for Unified Care to determine which roles best suit you and begin offering you shifts as soon as possible, we will require the following documentation:

1. 100 Points of Identification, which can be made up of the following:

• Current Passport —70
• Current Australian Drivers License — 70
• Birth Certificate (Original/Certified) — 70
• Credit/Debit Bank Card — 40
• Medicare Card — 40
• Centrelink Card — 40
• Tertiary Education Card —40
• Recent Utility Bill with Current Address — 40

2. Tax File Number
3. Bank Account Details
4. Superannuation fund and account number details
5. Working Visa (if applicable)
6. Original or certified copies of ALL relevant Diplomas, Certificates, Registration Documentation and Transcripts (where available)
7. Contact details for two relevant employment references
8. National Police Check Certificate
9. Immunisation record (for those seeking Hospital based employment)
